SWRSDC Competition
Waiver
I understand that it is the
intention of the 2007 Southwest Regionals Swing Dance Championships (SWRSDC)
to have my participation in the SWRSDC recorded on video tape, film,
and/or photographs, and I expressly agree to and grant the unlimited right
and authority to use any recording of my participation in the
aforementioned contest in any media, in perpetuity, in whatever manner and
by whatever means, without obligation to me.
Such recordings become the sole property of the persons or
organizations making them.
I
understand the physical risks of entering dance competitions and social
dancing, and assume full responsibility for any injury or personal
damages, resulting from such activities at SWRSDC.
I hereby, for myself and my heirs, executors and administrators,
waive and release any and all rights for damage I might have against the
SWRSDC and its agents for any and all injuries which may be suffered by me
in participation in dance competitions, social dancing and any traveling
to and from the dance convention locations.
As
a contestant, I understand that unsportsmanlike conduct or harassment of
officials or other contestants will be cause for disqualification or
suspension from current and future SWRSDC dance events and that refusal to
accept an award is considered unsportsmanlike
conduct. I certify that I am
18 years of age, or am represented by my Guardian below, if I am under 18
years of age.
ENTRY
FORMS MUST BE SUBMITTED TO STS THROUGH YOUR CLUB REPRESENTATIVE by April
25, 2007. This contest registration and competition waiver form MUST be
accompanied the $65 weekend ticket fee or proof of purchase.
